New Hire Paperwork

Complete the paperwork below on or before your first day of employment

In order to ensure accurate information is on file and for timely processing of payroll, it is very important that on or before your first day of employment you complete the below new hire paperwork.  

I-9 Form

Your job is contingent upon verification of identity and work authorization, I-9 Form. Complete Section 1 of the I-9 form and bring your original identity and work authorization documents (listed on page 3 of the I-9) to the Human Resources Office, room 203 Administration Building on or before your first day of employment.


HRS (MyUW) Portal Forms

The below forms can be completed by logging into the HRS (MyUW) Portal with your UW-Stout credentials and following the instructions below. To access the MyUW Portal, your UW-Stout account must be activated. Instructions to activate your account can be found on the New Faculty and Staff Technology Orientation site. Access to the MyUW portal is available up to seven days prior to your start date.

Direct Deposit

If we do not receive a completed direct deposit authorization with a valid bank account, payment will be made via a US Bank Focus Pay Card that will be mailed to the employee’s home address on file.

W-4 Form

Self-Identification Form

Transcripts (Faculty, Academic Staff, and Limited Employees only)

Official transcripts are needed for all degrees, not just highest attained. Please bring official transcripts to the Human Resources Office, room 203 Administration Building or have the degree granting institutions send electronically to askhr@uwstout.edu
