University of Wisconsin - Stout

Students in the Mental Health Counseling Program actively engage in counseling individuals while in various classes throughout the program. As a result, the Caregivers Background Check Law applies for students accepted into the program. In order to be considered for the program, you will be required to complete a Background Information Disclosure Authorization and Waiver and a Background Information Disclosure Record Form. Both can be found and printed from the UW-Stout website at http://www.uwstout.edu/soe/ugbenchmarks/ugbmi.html. Both forms will need to be submitted, along with the fifteen dollar fee, as part of the application materials. If you have difficulty downloading the forms, please contact us and we will have them sent to you.

The results of the background disclosure and record check will not constitute a condition of admission to the university. However, the results may affect the faculty's ability to place students in practicum and internship positions, therefore affecting program completion.
