Initial License Requests

Please use Form PI 1602 IS which can be downloaded and printed from the Wisconsin Department of Public Instruction website.  Complete the application form and mail it, along with the appropriate fee, to:

Note:  Fingerprint cards are required if you have worked, resided, or physically attended college class(es) in a state other than Wisconsin after age 17.  Please contact the CEHHS Dean's Office to obtain the DPI-required cards.

The Certifying Officer will request your transcript, endorse the application, mail it directly to DPI, and issue you a Verification of Program Completion form to show that the application is in process.

Form PI 1602 5R can be downloaded and printed from the Wisconsin Department of Public Instruction website, then completed and sent directly to DPI.  Official transcripts and license fee should be submitted along with the application form.

Renewal of a 5-year license for all professional school employees requires completion of a professional growth requirement in the five years preceding the begin date of the new license. Your license renewal requirements are detailed on your educator license certificate. Attach documentation to your application of six college credits of continuing education (original transcript or original grade reports) or PDP Verification Form.

For applicants who do not hold a current license because a previous license has expired, the six semester credits for renewal must be completed in the five year period prior to the effective date of the license renewal.

Note:  You have the option of renewal by successfully completing a Professional Development Plan (PDP).
