Affiliation Agreements for Clinical and Field Experience

Many departments, schools, and colleges require or encourage students to have clinical or field training as a part of the curriculum necessary to graduate. Such training experience is usually attained with the cooperation of an outside agency which agrees to participate in a cooperative effort with the particular school or college. It is in the interest of both parties that a formal agreement be in place prior to the acceptance of students into such programs. From a risk management perspective it is especially beneficial for the University department to negotiate a contractual agreement which clearly delineates the level of liability for the parties involved in a way that is consistent with our risk management objectives. Art Exhibit

If you plan to have an art exhibit, each artist or owner must submit a signed "Request for Art Exhibit Coverage" form along with an itemized listing of the works to be exhibited by name or description, and an individual value for each item. Should a loss occur, losses will be evaluated on the basis of the criteria on the art exhibit form.  Contact the Risk Management office for information  232-1793.

To determine the viability of any vendor our campus might wish to do business with, this website will tell you whether a specific vendor currently meets the University's insurance requirements for high-risk vendors.

Contractual Liability

Our office is available to review insurance clauses in contracts, and provide you with evidence of the University's insurance, in the form of a Certificate of Insurance or a letter explaining our coverage. Our office can also prepare waiver forms for your use, and can discuss your liability concerns.

Driver Authorization

Driving on University business requires a Driver Authorization form to be processed prior to travel or checking out a state vehicle. You will need to have this sent to the Safety & Risk Management Office, 7-10 days prior to travel to allow for processing.  This is necessary to drive a university vehicle and/or to be reimbursed for the use of your private vehicle. This form must be signed and sent to the Risk Management Office in University Services Room 130. Upon receipt, we will review your driving record and, if acceptable, we will sign the Driver Authorization form and return an approved copy to your supervisor. Approval or disapproval is based on the driving record at the time of the application. Any changes in this status must be reported to the supervisor immediately. Please also remember for out of state drivers, you must have your driver authorization form notarized.  This means you must present proper identification and sign such in FRONT of a notary.  Do no expect to send in the campus mail, and it will be automatically notarized.  Criteria for driving can be found on the UW System Web site.  Contact the Safety & Risk Management Office for more information.  Refer to State of Wisconsin driving manual for procedures and criteria for driving a state car.

Attention Student Drivers:   Student Driver Authorizations will be de-actived at the end of the Academic Year.  (May 31st)   .  Students that need to drive during the summer months and/or FY 09, must fill out a new driver authorization and submit to the Safety & Risk Management Office, University Services Building, Room 130.  If you should need any further information or assistance, please contact Safety & Risk Management Office at x1793

Special Events Insurance

Special Events Insurance is a system-wide insurance program that allows lessees and vendors who use University space and would not be covered under the University's liability policy to purchase this special event insurance. This policy is linked for your review.  Premiums are based on the type (hazard) of event and the number of those attending.  Please look over the program on the UWSA site.  Any questions, please contact the Safety & Risk Management Office, x1793 or x2258.

State Property Coverage provides physical damage insurance for campus buildings, contents, and moveable property including items in transit, art exhibits, borrowed and newly acquired property. Study Abroad and Exchange Program -- CISI

Study Abroad and Exchange Abroad program is insurance required for all students involved in academic pursuits while out of the country. This insurance provices a comprehensive package  of medical and emergency services specifically designed to address the risk associated with study abroad.  This program is also available to faculty and staff who are involved in educational programs abroad. Also, employees in work status while involved in educational pursuits abroad, have medical evacuation and repatriation through Medex .
