If your sport is not listed with any of the registration forms below, contact Intramural Sports to determine which registration form you should use.

Read all of the Sign up Information before filling out the forms or contacting Intramural Sports.

How to Submit the Forms

These forms are in PDF format; you will need to:

  1. Fill out relevant information (form fill-in).
  2. Print the form.
  3. Turn form in to the Intramural Sports Office (SFC room 41).
