HOW TO REQUEST A TRANSCRIPT

IMPORTANT: IF YOU NEED THE TRANSCRIPT SOON AFTER THE COURSE ENDS: Be sure to send your transcript request as early as possible. The end of the semester is a busy time period for the registrar's office. Send your request at least a couple of weeks prior to the end of the course if you need the transcript right away for recertification.

***On the request form check the box requesting that transcripts not be sent until grades are available for this semester. Be sure to list the course you are taking and the end date. Then the registrar will make sure that the transcript is NOT printed until after your coursework is completed and your grade is recorded on the Datatel system. Otherwise this could result in a transcript being printed without this semester's grade and credits listed.

TRANSCRIPT REQUEST FORM If an official transcript is needed for certification or documentation, the university registrar charges a fee for providing that service. The transcript request process and fees are available on the Registration and Records Office website. http://www.uwstout.edu/regrec/records.html

ORDER A TRANSCRIPT WITH PAYMENT BY CHECK: Order your transcript via mail with payment by check (not credit card payment). Download, print and complete this pdf form (requires Adobe Acrobat Reader) http://www.uwstout.edu/regrec/transcript_req.pdf
Please include:

  • Social Security or Student ID number
  • dates attended
  • your signature
  • the address to whom/where you want the transcript sent.

NOTE: Official transcripts cannot be issued to students with delinquent accounts.
