Initial License Requests

Please use Form PI 1602 IS which can be downloaded and printed from the Wisconsin Department of Public Instruction website.  Complete the application form and mail it, along with the appropriate fee, to:  School of Education, 267 HE, UW-Stout, Menomonie, WI  54751.

Renewals

Form PI 1602 5R can be downloaded and printed from the Wisconsin Department of Public Instruction website, then completed and sent directly to DPI.  Original transcripts and license fee should be submitted along with the application form.

Renewal of a regular five-year license is contingent upon completion of a continuing professional growth requirement of six semester credits.  The credits must be from an accredited college and be directly and substantively related to one or more of the licenses held by the applicant.

For applicants who do not hold a current license because a previous license has expired, the six semester credits for renewal must be completed in the five year period prior to the effective date of the license renewal.

Note:  You have the option of renewal by successfully completing a Professional Development Plan (PDP).
