1. DPI Scholarship Application

• Section I. Parent/Guardian signature required
• Section II. School Principal, Food Services Authorized Rep or DPI/WEOP Staff signature required
• Attach copy of most recent grade report or school transcripts
  

2. UW-Stout PreCollege Application

• Both student and parent/guardian signatures required
  

• Parent/Guardian signature required

4. Health History Questionnaire

• Parent/Guardian signature required on back of form
• Date of last tetanus booster required. If you don't know, you can obtain from your student's records at their school

5. Stout Adventures Form

• Student signature required on both pages
• Parent/Guardian signature required on both pages if student is under 18
