If you presently own a license plate with a disability sticker or have a Department of Transportation Tag, fill out Section A, copy your tag and mail, fax, or bring in to our office. If you do not have a State Tag, have your doctor complete all appropriate information in Section B and mail, fax, or bring in to our office.
