This card is used to give students permission to repeat a course which they previously received either a C, C+, B-, or B. Students can repeat courses which they received a C- or lower without this card, and may not repeat courses in which they received a B+ or higher under any circumstance.


The grade earned when the course is repeated will be used for computing the new grade point average; however, the original grade remains on the student’s record and will appear on all transcripts.


This card must be complete and signed by the student’s program director, and then turned in within five days after the start of the term to:

A Final Grade Report is different than an official and unofficial transcript. A final grade report is a reflection of one terms grades and one final grade report may be requested at the end of each term. However, if your full academic history at UW-Stout is desired then you may request an unofficial or official transcript.

There is no charge for an official grade report. A stamped, self-addressed envelope is required for each semester/term requested. Only one request per envelope will be processed. Failure to complete this entire form may result in a delay in the issuance of your grade report.

Due to the Family Rights and Privacy Act of 1974, your signature is required for the release of a grade report. Be sure to sign this form after printing it.

General Transcript Information

What is a transcript?

A transcript is a document that contains all of a students academic achievements at the University of Wisconsin-Stout. Transcripts are typically sent to employers, graduate school programs, and other academic institutions to reflect a students academic achievement.

Instances in which transcripts are commonly requested:

  • A student transferring to another college will typically need to send an official transcript to the new institution as proof of enrollment and as an indicator of academic ability.

  • Students often request official transcripts to be given to employers alongside a resume and other job application paperwork.

  • Unofficial transcripts do not fulfill the requirements of most schools and employers, and are often requested for personal student records. When requesting that an unofficial transcript be mailed, please include a postage-paid, self-addressed envelope.


Official Transcripts

An official transcript is printed on security paper and bears the signature of the Registrar.

  • Official transcripts may be mailed.

  • Official transcripts cannot be issued to students with delinquent accounts.

  • A minimum of 24 business hours is required for processing requests.


Official Transcripts can now be ordered online. Please visit our vendor, TranscriptsPlus, to see prices and enter your order.

Current students and recent graduates: You can expedite the process by logging in to Access Stout, navigating to your Student Center, and clicking on the "Request Official Transcript" link beneath the "other enrollment" drop-down menu. Follow these instructions to order an official transcript.

Note: All transcripts sent to the students are stamped "issued to student". Not all institutions (colleges, employers, scholarship funds, etc.) will accept these transcripts even if they are in sealed envelopes. It is up to the student to verify with the receiving institution if transcripts need to be sent directly from UW-Stout to them. Many institutions require transcripts be sent directly from UW-Stout to their institution to be considered official.

Unofficial Transcripts

An unofficial copy of the transcript is usually used for personal planning and advisement. It does not contain the signature or university seal and is printed on white paper.

  • Students are encouraged to pick up unofficial transcripts in the Registration and Records Office in person, if not available via AccessStout. Students who have access to unofficial transcripts via AccessStout are required to print their own unofficial transcript.

  • If a student cannot pick up their unofficial transcript, it can be mailed to the student at their current address. Requests of this nature must include a self-addressed, postage paid envelope.

  • Unofficial transcripts cannot be mailed to a third party, although it may be sent via inter-campus mail to an advisor or departmental office.

Once the Summer Session Registration form has been received and processed, a letter will be sent to out with further instructions for completing the Fee Payment Agreement, which is required before registration can begin.

Note: New students that wish to continue as degree seeking students in the spring must go on-line to https://apply.wisconsin.edu/ and complete the UW System application for admission.

If a course you are registering for requires a prerequisite you completed at an institution other than UW-Stout, then you must forward an official transcript to UW-Stout Admissions Office to confirm completion of the prerequisite.

High school special (non-degree) students must have their application for admission signed by one parent or guardian and a high school guidance counselor or principal. Transcript and fee are not required.

This form is used by instructors to drop students from their course who fail to attend the first day of the course and there are students waiting for open seats. Students who do not attend the first class meeting of a course, or who do not notify the instructor or department chair that they will be absent for special reasons, may, at the instructor's discretion, be dropped from that course if, and only if, other students are waiting to enroll at that time. Please note, however, that many instructors do not use this option. Do not assume that a course will be dropped automatically if you do not attend.
