Students in the Marriage and Family Therapy program actively engage in therapy with individuals, couples and families. As a result, the Caregivers Background Check Law applies. Students accepted into the program will be required to complete a Background Information Disclosure Authorization and Waiver and a Background Information Disclosure Record Form. Both can be found and printed from the UW-Stout website at http://www.uwstout.edu/soe/Documents/BackgroundCk.pdf. It is not necessary to complete these forms when applying; only when accepted. Both forms will need to be submitted, along with the fee. If you have difficulty downloading the forms, please contact us and we will have them sent to you.
The results of the background disclosure and record check will not constitute a condition of admission to the university. However, the results may affect the faculty's ability to place students in practicum positions, therefore affecting program completion.
