Students are encouraged to apply for the Dietetic Internship as soon as possible after completing nine graduate credits to facilitate internship planning. If required coursework has not been completed at time of application, acceptance will be predicated upon successful completion of these courses.
Complete a Dietetic Internship Application form and return to the Dietetic Internship Director, 225 Home Economics, University of Wisconsin-Stout, Menomonie, WI 54751. The Dietetic Internship application form instructs applicants to provide several items as listed below:

A standard verification form signed by the director of a didactic dietetics education program that is accredited by the Commission on Accreditation/Approval for Dietetics Education (CAADE),

Three letters of recommendation,

An official transcript of all graduate level course work; an undergraduate transcript may be required if requesting a waiver for a required course,

A copy of the letter of acceptance to the University of Wisconsin-Stout Graduate School,

Request the Graduate School to send the Graduate School Evaluation form to the Internship Director,

Obtain from Internship Director and complete a Background Disclosure Document and Department of Justice Criminal History Record Check,

For non-native speakers of English, TOEFL test score results sent directly from the Graduate School,

Dietetic Internship application fee of $45, check payable to UW-Stout.
Dietetic Internship applicants who are enrolled in the graduate program in Food and Nutritional Sciences must not participate in the computer selection process.
