If you are interested in applying to the Human Resource Certificate program, please print a copy of the application form, complete it and return it to to the address on the botton of the form.

To Register For Courses:
Step 1: Register online via Access Stout (if you have your student ID and have activated your UW-Stout email account) or complete a registration form.
- Current UW-Stout students use Access Stout at

- Not a current UW-Stout student, download a registration form from
htttp://www.uwstout.edu/outreach/ces/pdf/regform-credit.pdf
Complete the form and mail or fax it the contact information on the
back of the form.
Step 2: All students must have a signed Wisconsin Payment Plan Agreement on file with Student Business Services before you can register for UW-Stout courses. You may access this form at:
http://www.uwstout.edu/stubus/PDF/ppagree.pdf
Mail or fax it with your registration form.
Current students can also submit the Payment Plan Agreement via Access Stout at: https://access.uwstout.edu/access/webadvisor
It is recommended you register as soon as possible so you are able to enroll in the courses you need and receive course materials and textbooks prior to the course start date.
