Video Booking Request Form
Use this form to have UW-Stout videocassettes or DVD's held for instructor's classroom use.
- Booking requests must be submitted a minimum of two weeks prior to the planned pick-up date.
- Videocassettes or DVD's "booked" for class may be picked up at the Circulation desk. If you have any questions, please contact Circulation staff at 715/232-1215 or by email: circulation@uwstout.edu
- Notification will be emailed verifying receipt of the request.
