Baseline Plus Change Request
This form can be used to either increase or lower your Baseline Plus dollar amount for second semester.
Baseline Plus funds are used in laundry machines, copiers, printers, vending machines, Bookstore, Health Center, Rec Center, Health & Fitness Center and other campus locations.
To view the amount of funds you started first semester with, log into your Access Stout account and click in the Financial Information area. Choose Archived Billing Statements and select the August 2007 statement. Log in as indicated. Look for the line that begins with “IDFUN”; this is the amount of Baseline Plus funds that you started first semester with.
If you made additional deposits during first semester, take the IDFUN amount and add your additional deposits to get the total amount of funds that was needed for first semester.
