Family Planning Waiver Program Brochure

FPW Brochure :  This brochure encompasses all the information you need to know about the program such as who should consider this program, what services are included, and what criteria you have to meet in order to qualify for the program.

To ensure the application process is as smooth as possible please use the following checklist as a guideline.
Before you get started you will need:

1. Computer with internet access
2. Access to printer
3. Proof of Identity (driver's license, student ID card)
   
4. Proof of Citizenship (by way of copy of your birth certificate or a copy of your passport if you were not born in Wisconsin).
5. Income verification (check stubs for 30 days prior to application).  Note - Work Study is not considered for this program and therefore you do not have to provide income verification. Any other source of income (State payroll, etc.) will need to be revealed.

1. Click on "Apply for Benefits"
2. You will be required to create a Wisconsin User ID and password to access your account.
3. Provide the necessary information as required by the application.
4. Upon submission of your application you will be directed to a confirmation page where you will receive a ten-digit tracking number. VERY IMPORTANT: In order to receive services covered under the Family Planning Waiver at Student Health Service you MUST print and turn in a copy of this page to our office.
5. IMMEDIATELY mail  (picture ID, birth certificate/passport, paystubs-include Social Security # and ten-digit tracking # on these documents) to Enrollment Services to expedite the processing of your application.

Failure to provide above documents results in ineligibility for services and you will incur the charges for services rendered - cost of services range from $20.00 - $230.00 (subject to change).
