Counseling Forms
- Counseling registration forms. Below are instructions for completing 2 forms prior to attending for your first counseling appointment. Please contact our office to schedule an appointment before attempting to complete these forms.
1) The first link is to the Client Information Form (Requires Adobe Reader). Please fill it out, print it and bring it with you to your first appointment.
2) The second link is to a Confidential Symptom Questionnaire. After opening this link, follow the steps below to login and complete the questionnaire (these steps are repeated on the website):
a) Click Student Login on the right side of the screen.
b) Enter a User ID and Password. Your User ID is your 7 digit student ID number, which will begin with a 0. Your Password will be pre-set as your last name, first initial of your first name, and 2005--all in lower case and without spaces (for example, if your name is Jon Doe, your password will be doej2005). You may follow online instructions to change your password later if you wish.
c) Click the Login button
d) Read directions and click on Click here to begin your Assessment.
e) Scroll down to Outcome Questionnaire, answer questions and select finished click here . You do not need to print the report, as we will print it in our office and give it to the counselor you are scheduled to see.If you have questions or experience problems, please call 232-2468 for assistance.
- Release of Information Form. For authorizing release of confidential information from counseling records. Hard copy must be completed and signed by clients and either mailed or faxed to our office.
